Pre-registration Form
The Boonshoft Museum is proud to offer engaging, interactive animal programs through our Discovery Zoo! In order to expedite and streamline the registration process, we ask that you fill out this pre-registration form that includes your preferred dates for your Animals Alive! program, and provides us with all the details needed to book your program quickly and easily.
Please note that this form is a request only! Submitting this form does not guarantee or "hold" dates, as specific dates are based on staff availability. A staff member will get back to you within two (2) business days to finalize plans for our visit to your location, so please be sure to include accurate contact information.
For the best experience for your students/participants, please note that the Animals Alive! program works best in a small group setting. Our presenters cannot provide a quality interactive experience for participants in an auditorium setting (eg: on a stage with a microphone).

Tell Us About Your GroupRemember! Preschool & Pre-K group programs are 45 minutes, with a maximum of 30 students per program. For all other groups, programs are 60 minutes, with a maximum of 50 participants. Teachers should plan to remain in the room throughout the program.
The Boonshoft Museum makes every effort to create a positive and unique experience for program participants. While we cannot guarantee availability of any specific animal ambassador from the Discovery Zoo, please let us know if you have any requests in terms of the types of animals that visit. Any curriculum topic(s) that you would like to see incorporated, if possible?
