STEM-To-You Program Request Form

The Boonshoft Museum is proud to offer engaging hands-on opportunities in your own classroom. In order to expedite and streamline the registration process, we ask that you fill out this request form. This form will allow you to fill out all your essential information, pick 3 possible dates for your program, and provide us with all the details the Museum’s Education Staff needs in order to book your program quickly and easily. Requests must be made at least THREE WEEKS in advance.

Please note that this form is a request only! Submitting this form does not guarantee or "hold" dates, and specific dates and workshops are based on staff availability. A staff member will get back to you within two (2) business days to finalize plans for our visit to your classroom, so please be sure to include accurate contact information.

This program will take place at a DIFFERENT LOCATION than the organization address listed above.

For Preschool (ages 3-5) and Kindergarten programs, presentations are 45 minutes and have a maximum capacity of 25 students per class.

For grades 1-12, presentations are one hour and have a maximum capacity of 30 students per class.

For animal programs, Preschool & Pre-K group programs are 30-45 minutes, with a maximum of 30 students per program. For all other age groups, programs are 45-60 minutes, with a maximum of 50 participants.

Exceeding these numbers will result in additional fees.

Teachers should plan to remain in the room for all programs.

Remember: For the best experience for your students/participants, please note that the Meet the Animals program works best in a small group setting. Presenters cannot provide a quality interactive experience for participants in an auditorium setting .

I understand the Boonshoft Museum policies and guidelines in terms of group size, fees, and other group restrictions. Further, I understand that exceeding these numbers will result in additional fees being assessed.

In addition, I understand that mileage will be assessed for programs in locations more than 25 miles from the Museum, and that this will be confirmed at the time of booking. Maximum range for programs is 40 miles.
